TOKYO, Japan - Transport minister Kazuo Kitagawa (R) receives an interim report on the April 25 fatal derailment on JR West's Fukuchiyama Line from Aircraft and Railway Accidents Investigation Commission Chairman Junzo Sato on Sept. 6. The reports said that West Japan Railway Co. trains experienced 46 incidents of an advanced automatic train stop system being activated due to driving errors or other reasons in fiscal 2004.

TOKYO, Japan - Land, Infrastructure and Transport Minister Chikage Ogi (L) receives a final report on the near-miss incident between two Japan Airlines (JAL) passenger planes in January 2001 from Junzo Sato, chairman of the Aircraft and Railway Accidents Investigation Commission on June 12. The report concluded that air traffic control's error in giving the wrong flight numbers when asking the pilots to change course and the pilots' decision to follow air traffic control instead of the computerized Traffic Alert and Collision Avoidance System (TCAS) were two of the main causes.
